What changes once a stream is live

Before a stream starts, its thumbnail is whatever cover image the creator set for the scheduled broadcast. Once it's live, YouTube may periodically refresh the auto-generated preview from the current broadcast.

If you download a thumbnail mid-stream and it looks slightly stale, that's expected — YouTube's CDN caches these images briefly rather than updating them frame-by-frame. Once the stream ends and becomes a normal on-demand video, its thumbnail behaves exactly like any other upload.
